Each bearing should have one spacer set and one adapter set, so each shock should come with 8 individual spacer/adapters total.
One set is a spacer and allows you to use a 5/8" bolt.
One set is an adapter and allows you to use a 1/2" bolt.
Both sets are 1.25" wide.

You do not have to use our spacer/adapters. No matter how you mount them make sure the bearing is free and allows the shock to move. If you bind the mount you'll break the shock.
I've seen the shocks bolted directly to the frame, so nothing can move. This is not good!
