Looking for advice on wheel & tire size for a 87 Buick Regal. Thinking 17" or 18" with a 4" backspace front and 4.5" rear.

Your backspace settings are right on IMO.
I have an 84 Monte SS, with 17x4 and 17x9.5, 0 mm offset front/rear, Nitto 555, front: 255/45/17
Rear: 285/40/17, with 1/4” spacers on the rear (I replaced 275s for 285s).
I suggest using something like UMI Performance adjustable Roto-joints to get the rear axle centered, there is very limited space, the Monte rear wells are 11” from fender lip to the tub wall.
The 9.5” rear wheels are measured on the inside mounting lip, ending up at or around 10.5” externally.
Depending on your wheels and bolt pad depth, you may need longer wheel studs.
(i bought some Billet wheels and the rears were a few mm thicker)

I have 17's on my 87 Regal. Keep in mind the Monte's have more room in the front.
Front: 17x8 4.5'BS 225/45/17
Rear: 17x9.54.75"BS 275/40/17
