That's an on-board hard drive for storing your music on. Some of these load your music automatically as you listen to the CD. Some also have the CDDB data base so the display will show the artist and song title as long as it's in the data base. Would probably be hard to incorporate into an older car, the audio components are all networked together and communicate with each other.
