I spent the last hour searching this website and the internet and I think I'm more confused now than I was when I started. Basically where should the tube I'm pointing to with the screwdriver be hooked to? Somewhere on the air intake between the air filter and throttle body?

Also, my rear drivers side port is capped. Is that correct?

its for a LS3

You tee the two valve cover ports together and then connect them to a port you add to your intake tube between the MAF and throttle body. That way the fresh air is metered by the MAF.

That port goes to the air intake tube (after maf, as mentioned) or throttle body port (if one exists , on a DBC TB, but yours is a DBW TB, so would be air intake tube)
see pics

hope this helps
